Terms and Conditions

I confirm that I am experiencing genuine financial hardship and require assistance from The 4S Legacy Inc to access a private pet cremation and memorial keepsake package.

I confirm that I have exhausted all reasonable financial support options available to me, including assistance from family, friends, savings, payment plans, or any other available resources. I acknowledge that assistance from The 4S Legacy Inc is my last remaining option.

I agree to provide any supporting information or documentation reasonably requested by The 4S Legacy Inc to verify my financial hardship or any information provided within this application.

I declare that all information provided in this application is true, complete, and accurate to the best of my knowledge. I understand that providing false, misleading, or incomplete information may result in my application being declined, withdrawn, or any approved assistance being cancelled.

I further acknowledge that if it is determined that I was not experiencing genuine financial hardship at the time of my application, or that I knowingly provided false or misleading information, The 4S Legacy Inc reserves the right to recover from me the full value of any financial assistance provided on my behalf.

I authorise The 4S Legacy Inc to liaise directly with my veterinary clinic and/or the nominated pet cremation provider for the purposes of confirming information, arranging services, and making payment on my behalf.

I understand that assistance is subject to available funding, eligibility criteria, and the availability of charitable resources. I acknowledge that submitting an application does not guarantee approval.

I understand that approved funding is paid directly to the nominated pet cremation provider. No funds will be paid directly to the applicant.

I understand that assistance provided by The 4S Legacy Inc is strictly limited to a private pet cremation and memorial keepsake package only. Additional services, upgrades, products, viewings, witness cremations, or any other optional extras cannot and will not be funded or provided by The 4S Legacy Inc.

I understand that any veterinary clinic holding, boarding, storage, after-hours, or other fees incurred before my pet is collected by the cremation provider are the sole responsibility of the pet owner. The 4S Legacy Inc is not responsible for these costs and they will not be funded as part of this assistance.

I understand that The 4S Legacy Inc reserves the right to decline any application that does not meet its eligibility criteria, where sufficient evidence of financial hardship has not been provided, where funding is unavailable, or where the information supplied is considered false, misleading, or incomplete.

I confirm that I have read, understood, and agree to these Terms and Conditions and acknowledge that the decision of The 4S Legacy Inc regarding my application is final.
